The survey can be completed on any computer or mobile device. Please note: You may pause the survey and return to it at a later time. A pause button is located at the bottom of every page after Section 1. When you are finished with the online survey, you MUST click "submit" to save your answers. The screen will tell you that you have completed the survey. If you do not see this screen, your survey has not been submitted, so you will need to start over.
